How to apply for replacement of lost TIN Card (Updated 5/20/2010)

First, visit the BIR District office where you acquired your TIN Card.

If you don’t know which BIR District Office you got your TIN Card. You can call the TIN Verification Office of BIR 926-8339 or the BIR Contact Center 981-8888 or you can EMAIL at contact_us@cctr.bir.gov.ph  give them your full name and your birthdate.  Just ask them that you want to know where your TIN is currently registered or which BIR District Office your TIN is registered.   

For the the Address of a particular BIR District Office you look it up here .

That is the BIR directory of the district offices of BIR.  Or you can call the BIR Contact Center at 981-8888.

Get the address of the BIR District Office where your TIN is registered. The  BIR Form 1905 is what you will be using

Click this link so you can see the actual form BIR Form 1905.  In part II of this form “REASON FOR REGISTRATION INFORMATION UPDATE” in section 4B “REPLACEMENT OF LOST / DAMAGED TIN CARD“.

The needed attachment for this application is an Affidavit of Loss duly notarized of course. You can get this service from a Notary Public near your area. The notary usually cost around 50 to 100 pesos. For replacement just present your old TIN Card.

Don’t forget to sign the backside of this form.  After you submitted your application ask the BIR personnel when you can get your TIN Card.  Lastly, where do you file this? Submit this 1905 to the BIR District office where your TIN is currently registered.  If you TIN is registered in BIR RDO handling Marikina then thats where you should file that request not with any other BIR district office. That’s it.

For new applicants for TIN card there is nothing to pay. But i came to know recently that they are charging for replacement. due to loss, around 100 pesos.

Gawin ninyo lang po ang sinasabi ng blog post ko ukol sa pag apply ng bagong TIN Card na nawala. Una lang alamin mo kung saang BIR District office naka rehistro yun TIN number mo at dun ka pupunta para mag-apply ng bagong TIN. Salamat po sa pag dalaw sa aking blogsite.

Eh panu po kung yung place ko na naka register ako ay Bacolod city tpos nandito po ako sa manila.hindi ko po makuha?

Kung naka register po kayo sa Bacolod, then puwede po ninyo ipalakad sa inyong kamag-anak o kakilala run. Ipadala ninyo lang yun accomplished form ng 1905. But i suggest to request to transfer your TIN to the RDO office who has jurisdiction over the area where you are currently employed. 1905 is still the form you will be using April. On the bottom part of that form sign the back portion make 3 copies and send it to any of your relatives in Bacolod to submit it for you. It usually takes an hour to route to the sections in the RDO. You can then check it with BIR TIN verification office if they have transferred your TIN. Kung na transfer na dito sa Manila. Punta kana sa District office na yun at apply ka naman ng TIN ID.

Another option would be to fax the 1905 form to the BIR Office in Bacolod. the TSS section is the one handling this.

I never had a TIN card. All I have with me is my TIN number. How do I get a TIN card? I believe it is one of the national valid IDs.

fill out the BIR form 1905. Like the article on this page this is the steps you will take. Verify where your TIN is currently registered or which BIR District office issued that TIN. This is where you should request for your TIN card using the 1905 fom. In this article there is a link on a PDF copy of the BIR form 1905 print this out. There is a portion in that form where you can write what you need. write down TIN CARD. Submit that to your BIR District office.

Sir ask ko lang kasi as of now unemployed ako, meron ako TIN ID pero nawala. gusto ko sana kumuha ng bagong TIN ID kasi im planning to open a personal bank account. tumawag ako sa BIR Quezon City pero as per agent na nakausap ko. Hindi daw pwede kumuha ng NewTIN ID dahil unemplyed daw ako. Ano kaya maganda gawin?

Kung meron na po kayong TIN sa BIR. Kelangan lang po ninyo malaman kung saan ito kasalukuyang naka rehistro. Tumawag po kayo sa BIR contact center 981-8888 or 981-7000 local 7030 or sa local 7251. Alamin mo kung saang Revenue District Office (RDO) ng BIR naka rehistro itong TIN mo. At dun kayo pumunta. Gamitin mo ang form 1905. Basahin mo ng mabuti ang article sa itaas para sa karagdagang info para gawin ito. Hindi po totoo na kung wala ka ng trabaho e hindi kana puwedeng mag request ng TIN card. Pagkat meron ka ng rehistradong TIN sa BIR puwede po kayong magrequest uli ng TIN card.

Depende po sa workload ng RDO po ninyo. Kaya mabuti po na makuha ninyo ang contact person na tumangap ng inyong application. But usually naman kung hindi naman ganun karami po ang load nila makukuha ninyo agad on the same day. Ibibigay lang po sa inyo ang TIN card ninyo, kayo na po ang bahalang maglagay ng pic at magpa laminate ng inyong TIN card.

sir ask ko lang po about sa verification ng RDO..
tulad po nila nawala din tin card ko nung bagyo..
dbale po kanina lang nagpaverify me sa BIR dun sa makati, tapos nilagay sa form ko ung RDO ko tapos sa Quezon city daw, kaso nga lang ala “tatak” or seal?
anu po ibig sabihin nun?
sa bir quezon city me mag paverify?

another question po… may online verification po ba ng RDO sa website ng BIR….

Hindi po talaga tatakan yun ng BIR Makati dahil hindi naman sila magpa-process po ng inyong application, na verify po nila siguro kung saan currently naka rehistro ang inyong TIN kaya dun po kayo pupunta. Sa BIR Form 1905 may section 4B “REPLACEMENT OF LOST / DAMAGED TIN CARD“. Check ninyo yun box na LOST TIN CARD. Ipapasa ninyo po yan sa BIR District ng QC. Kung RDO 39 SOUTH QC yan ay nasa may malapit sa DELTA THEATHER sa may QUEZON AVE. corner SCOUT SANTIAGO nasa loob po yun ng REGIONAL OFFICE NG BIR QC - REGION 7. Click ninyo ang LOCATIONAL MAPs link sa gawing bahagi ng blog para makatulong sa pagpunta ninyo. Kung RDO 38 sa may WEST AVE, harap ng ST. VINCENT SCHOOL malapit sa Mcdo. Wala pa pong online verification sa website ng BIR.

pwede po magtanong? 2005 pa po nvng last ako mag trabaho, wala ako tin card, at nawala yung form ko na ni fillup ko noong nag register ako sa BIR pampanga, paano ko makukuha yung tin no. Ko kung nawala ko yung form? Sna po paki sagot, o email nyo po ako kenneth.boydon@yahoo.com

Hi Ihajahm, sa BIR TIN Verification office po or sa BIRCC. Ang mga telephone numbers po nila ay nakalagay po sa article sa itaas. Tawagan ninyo lang po sila para malaman ninyo kung valid ang TIN number po nila.

Download and fill up Form 0605 then pay P100 at BIR-accredited banks. Bring the bank-validated Form 0605 with you when you go to BIR otherwise you’re going to waste a lot of time. You need 3 copies of FOrm 0605.

When I walked into the BIR-Lucena office, there was a fairly large sign on the 4th window that told clients to get a number from the guard and “wait for your turn.” I waited 2 h and 15 min only to be told that I have to go to a bank first to pay the P100 fee. That was annoying. I wondered where their instruction was that a client has to go to a bank first. They did have an instruction posted - on an index card, typewritten in 12-type font under the large sign that told clients to get a number from the guard and “wait for your turn.” Talk about inefficiency! These chattering employees could have spent their time better by posting procedures in large font where they are easily seen BEFORE instructing clients to “get a number and wait for your turn.” There are no instructions on the BIR website that tells clients what the standard procedure is otherwise I would have expected that I would pay and I would have asked beforehand where I should pay instead of waiting for over two hours for the chattering employees to call my number and tell me that I should go to bank.

To add to my annoying afternoon, there were 40+ clients waiting for their turn and only two employees (out of the many many chattering employees) actually calling out numbers and attending to clients.

Hi Arturo, you can easily get this information by calling the numbers i provided in this article. Either the BIR Contact Center or the BIR TIN Verification ofc. can give you this information. Frankly, i do not have access to that facility. That is why i provided the contact numbers indicated in this article for people to get the information they need pertaining to their TIN. Hi. I applied for TIN number last 2002 and after a week BIR issued a TIN card. I was advised then that the digitized TIN card will be mailed to my address. I has been 8 years and I haven’t receive any digitized TIN card. Can i still get my digitized TIN card? Does BIR still issue digitized TIN card? Whenever I do transactions and present the TIN card issued by BIR i am being asked for the digitized BIR TIN Card. How can I get a digitized TIN Card?

Hi Ronna, if your referring to the plastic card. The project has been stopped by our government. And was not provided the budget to continue this I believe.

The TIN Card being issued presently by BIR is the old thick paper type of card. So the owner of the TIN are the ones who will provide the photo and laminate it for better protection against the elements.

You can get this thru the BIR Revenue District Office where your TIN is currently registered. Use 1905

hi po..ask ko lng po kc nawala un tin number and sss ko..di ko po kasi matadaan mga number nila..kaso nandito po aq sa gensan pwede po kaya aq makakuha ng tin number ulit or tin card?alam ko po kc sa manila ko po kinuha un tin number ko..naasign po kc aq d2 sa gensan..kailngan ko ngaun ng tin number,saka sss pa..pwede po kaya aq kumuha d2 sa gensan?plz response po..thanks

Hi Aiza, I can only answer regarding your TIN. I cannot answer you with regards to your SSS, for its a separate from BIR. You might want to visit their local office there in Gensan with regards to your SSS. Or visit their website www.sss.gov.ph

Request for TIN Card can only be done by the BIR Revenue District office (RDO) where you TIN is currently registered. So the BIR office in Gensan can view your registration but they cannot process any application pertaining your TIN since it is with the BIR RDO office in manila as you said.

But you can get your TIN number by emailing the BIRCC. EMAIL at contact_us@cctr.bir.gov.ph

Or you can fax a request to transfer your TIN in Manila to Gensan RDO. So you can request a TIN Card with the RDO in Gensan once its transferred.

But again if you are transferred back to Manila, you should make sure that you apply for transfer of again of your TIN so you won’t have any problems like this.

With regards to Postal ID, hindi ko po masasagot ito dahil hindi po ako taga Philippine Postal Office. Maari po kayong magsadya sa Postal Office diyan upang malaman ninyo ang mga requirements nila.

Pls. kindly help us po kasi yung mother ko ngwork before sa Queens Supermarket around 1980’s tapos nasunog po sya at naiwan nya yung i.d. sa company after that nagdecide po sya tumira kung nasaan ang tatay ko at hindi na po sya ulit ngwork sa company but instead ngtayo na lang po sila ng maliit na sari-sari store. Ngayon po ngwowork po sya sa isang cooperative dito sa amin as Treasurer at required po ng bangko nila yung tin number kasi po signatory sya ng tseke kun kaya’t malaki po problema nya kase po hindi nya po saulo yung tin number nya at wala na ring po yung i.d. nya dahil sa sunog noon kung kaya humihingi po kami ng tulong sa inyo kung papaano namin malalaman ulit yung tin number nya kung kaya ngbabakasakali kami sa email na ito na inyong sasagutin, hindi naman po kami makaluwas kasi hindi sapat yung budget at hindi rin po sya pupuwedeng mag-absent. Almost two weeks na po ako nag-email sa BIR kya lang po wala pa rin silang sagot kaya sa inyo po ako nagbabakasakali. Eto nga po pala yung pangalan nya nong dalaga TERESITA BIOC ADELIN, Birthday: October 03, 1951. Sana po matugunan nyo ang aming munting kahilingan, kasi kailangan na po nya dahil hinihingi na po sa kanya ng bangko. Maraming salamat po. Magandang araw!

Hi Norina,

Para po sa inyong kaalaman ang mga ginagamit po nun ng BIR sa gayong panahon 1980’s ay TAN pa po hindi TIN. Mga 1990s pa po na-implement ang gayong pagbabago.

Kaya malamang po TAN po ang hawak ng mother mo nung mga panahon na yun na matagal na pong tinangal itong TAN sa system ng BIR.

Ang lahat po na may TAN nun ay pinag-apply ng TIN. Kung gusto po ninyo maka siguro ay kayo mismo dapat po ang tunawag TIN verification office. Wala po akong access sa database ng BIR.

Ayoko ko pong isipin ng iba na ang blog na ito ay maaring gamitin sa TIN Verification. Pasensiya na po pero wala po akong access dun. Kaya po nag provide po ako ng numbers sa article na ito upang makatawag kayo sa opisina ng BIR na meron access sa TIN database.

Pag na confirm na ninyo po sa TIN Verification office ng BIR 926-8339 or sa BIRCC 981-8888, at naputanayan po natin na wala po siyang TIN. Maari naman po siyang mag-apply ng TIN sa BIR na may hawak po sa area ng pinagtatrabahuhan ng inyong mother.

At since employed naman po ang inyong mother. May article po dito sa pag-apply ng TIN upang malaman ninyo ang proseso ukol dito. Ito po ang link sa ibaba.

Need help lang po, kasi my Tin Id was processed by my previous employer, kailangan pa ba nang ibang forms to fill out or may additional things to process para ma-transfer sa new employer ko?

Sabi kasi nang Human resource manager ng new employer ko, hindi nag-iisue nang replacement for Tin ID. Which I doubted.

Hi Jove,

Contrary po sa sinabi ng HR Manager mo sa iyong new employer, as the article informs us na puwede po tayong mag-apply for replacement for Lost TIN Card.

Or if you haven’t requested yet for a TIN card then yun form 1905 pa rin po ang gagamitin natin para po mag apply ng TIN Card. On Part II of that form check “K” and box provided write down “TIN CARD”.

Of course you have to submit this to the BIR Revenue District Office (RDO) where your TIN is currently registered.

Now with regards naman po dun sa pag transfer ng TIN just read this article and it will guide you thru the process of transferring your TIN. Same form will be used. Again this will be submitted only to the RDO where you TIN is currently registered. Click the link to the said article:

Hindi ba pwede mail nalang ng BIR yung Tin Card sa address namin katulad ng sa NSO birth certificate para naman hindi na mahirapan kaming mga kukuha nitong Tin Card na ito?

Sir/Mam patulong naman po, kasi po nabasa sa baha yung copy ko ng 1902, ngaun po hinahanapan po ako ng bago kong employer ng proof na meron nga po ako tin number, tsaka isa po sa mga requirements nila yun, ngaun po problema ko kung makakakuha po kaya ako ulet ng copy ng 1902 ko sa bir? hintayin ko po sagot nyo sir/mam. salamat po, god bless
