First, visit the BIR District office where you acquired your TIN Card.
If you don’t know which BIR District Office you got your TIN Card. You can call the TIN Verification Office of BIR 981-7000 local 7030 or the BIR Contact Center 981-8888 or you can EMAIL at contact_us@cctr.bir.gov.ph give them your full name and your birthdate. Just ask them that you want to know where your TIN is currently registered or which BIR District Office your TIN is registered.

Get the address of the BIR District Office where your TIN is registered. The BIR Form 1905 is what you will be using
Click this link so you can see the actual form BIR Form 1905. In part II of this form “REASON FOR REGISTRATION INFORMATION UPDATE” check the letter “K” box and just below it write down what you need which is a “TIN CARD”.
The needed attachment for this application is an Affidavit of Loss duly notarized of course. You can get this service from a Notary Public near your area. For replacement just present your old TIN Card.
Don’t forget to sign the backside of this form. After you submitted your application ask the BIR personnel when you can get your TIN Card. That’s it.
For new applicants for TIN card there is nothing to pay. But i came to know recently that they are charging for replacement. due to loss, around 100 pesos.

Ipadala ninyo lang yun accomplished form ng 1905. But i suggest to request to transfer your TIN to the RDO office who has jurisdiction over the area where you are currently employed. 1905 is still the form you will be using April. On the bottom part of that form sign the back portion make 3 copies and send it to any of your relatives in Bacolod to submit it for you. It usually takes an hour to route to the sections in the RDO. You can then check it with BIR TIN verification office if they have transferred your TIN. Kung na transfer na dito sa Manila. Punta kana sa District office na yun at apply ka naman ng TIN ID.

Verify where your TIN is currently registered or which BIR District office issued that TIN. This is where you should request for your TIN card using the 1905 fom. In this article there is a link on a PDF copy of the BIR form 1905 print this out. There is a portion in that form where you can write what you need. write down TIN CARD. Submit that to your BIR District office.
